Please prepare the following before enrolling to Junior High School:
  1. Original Copy of Form 137 from Graduated Elementary School.
  2. Good Moral Character.
  3. Original Copy of Form 138-A (Report Card).
  4. Xerox copy of Birth Certificate from PSA.
  5. Accomplished Registration form

Senior High School

  1. Original copy of Form 138-A (Report Card).
  2. Good Moral Character.
  3. Form 137 From Graduated Secondary School.
  4. Xerox copy of Birth Certificate from PSA.
  5. Accomplished Registration form.
Note: Failing in submission of all the requirements will be enrolled temporarily.
